Lab note

In plain words

Over 2-12 weeks the expected extra weight loss is about 0.9-1.3 kg at most versus placebo, below the 2.5 kg threshold one meta-analysis used for clinical significance, and none in the largest RCT.[1][2][3][4]

Claim by claim— each claim graded on its own

At most about 1 kg: pooled estimates of -0.88 kg and -1.34 kg, while a 12-week JAMA RCT and a 54-trial herbal meta-analysis found no effect.[1][2][3][4]

Two meta-analyses show small, borderline effects of uncertain clinical relevance, but the largest well-run RCT was null and another meta-analysis found no effect. Mixed and weak, so Grade C.

Each plate is one paper. Taller = stronger study design, wider = more studies pooled. Tap one.

20′11′20′98′

Mixed: one small single-blind crossover found 15-30% lower energy intake, but a double-blind crossover found no change in satiety or intake.[5][6]

Only small, short (2-week) crossover trials exist and they disagree. Grade C.

Each plate is one paper. Taller = stronger study design, wider = more studies pooled. Tap one.

How it works

Garcinia cambogia (Garcinia gummi-gutta) is a tropical fruit whose rind provides hydroxycitric acid (HCA), the ingredient standardised in weight-loss supplements. It is not a normal part of most Western diets, and the opened sources give no dietary intake figure.

HCA competitively inhibits ATP-citrate lyase, an enzyme needed to make new fat from carbohydrate (de novo lipogenesis). It is also proposed to reduce appetite. In people, a 12-week trial found no extra weight or fat loss, and appetite trials disagree.[4][5][6]

Dose

Trials used 500-1500 mg HCA/day (500 mg/day, 300 mg three times daily, or 1500 mg/day in the JAMA trial)

  1. 500 mg HCA/day, 2 weeks

No change in satiety or energy intake.[6]

  2. 900 mg HCA/day (300 mg x3), 2 weeks

    24-h energy intake 15-30% lower in a single-blind crossover.[5]

  3. 1500 mg HCA/day, 12 weeks

No extra weight or fat loss versus placebo.[4]

  4. Across trial doses

Nonlinear association between dose and weight change.[2]

Lab note

Timing. Trials gave HCA in divided doses with or before meals; no timing has been shown to matter.[4][5]

16 of 22 registry liver-injury cases involved garcinia with green tea; the injury was clinically indistinguishable from green tea alone.[7]

Safety

Caution
Pooled trials show no significant change in liver enzymes, but a US liver-injury registry adjudicated 22 serious cases from garcinia (most combined with green tea), with 91% hospitalised, one death and one transplant. GI complaints were twice as common as placebo in one trial.
Possible side effects
  • Gastrointestinal upset
  • Rare severe hepatocellular liver injury with jaundice
Talk to a doctor first if
  • You have liver disease
  • You are taking it with green tea extract or other fat-burner ingredients
  • You are pregnant or breastfeeding

Myths vs research— scroll through

Myth

Garcinia blocks fat so you lose weight without trying

What the research says

With diet, HCA produced no extra weight or fat loss over placebo in a 12-week JAMA trial.[4]

Myth

Normal blood tests in trials prove it is liver-safe

What the research says

Pooled trials showed no ALT or AST change, but a registry documented rare severe liver injuries including a death.[7][8]

FAQ

Pooled analyses suggest about 0.9-1.3 kg more than placebo at best, and some analyses find none.[1][2][3]

One small trial found lower food intake; another found no change in satiety or intake.[5][6]

In registry cases, injury appeared 13-223 days after starting (median 51), usually with jaundice.[7]

  • Grades are judged for healthy adults who train; no trials in trained people were found.
  • Vuppalanchi 2022 is a registry case series (typed "review" because the format has no observational type); it shows that serious injuries occur, not how often.
  • Westerterp-Plantenga 2002 was single-blind; Kovacs 2001 was double-blind and found no effect.
  • Individual case reports of garcinia hepatotoxicity (e.g. PMID 31879192) were seen as titles only and are not cited.
  • No opened source quantified contamination of garcinia products with prohibited substances.
